A Deed Poll is said to be "executed" when it is filled in by you after you have received it. To execute a Deed Poll you must: Sign it in the presence of two witnesses. The witnesses should not be related to you. Each witness must also sign and date the Deed Poll to agree that they have witnessed your signature. In addition for children, signatures are required from all those with parental responsibility.
