A woman may revert to her maiden name on being widowed. Generally,
the marriage certificate and her husband's death certificate will
be accepted by all parties as sufficient evidence of the change
of name.
If they are not accepted, a Deed Poll will be required.
If a man who previously changed his name by Deed Poll when he got
married wants to change it back on being widowed, he will have to
use another Deed Poll to change his name again.
